Allow Dates and Comments

It's been suggested that it could be helpful if users could see start/end dates and comments on (similar functionality to previous group memberships) current group memberships fields. This was requested as some constituents have left/rejoined the groups and users would like a quick option to view this.

    This is to add start date and comment to the " Constituent Current Constituent Group Membership List" data list. If the idea to allow future end dates to be added to group membership, then also display the end date field on this data list.
