Allow Start and End Date to be entered for attribute extension form

I'm submitting this request on behalf of ELCA. It's great that an attribute extension form can be created and attached to an existing data entry form. However, when you do this you cannot enter the start or end dates for the attribute value. The system only allows the user to enter the attribute value. Attribute extension forms should include the value, start date and end dates fields.

    Hi Lisa, at present we are focused on a number of items with higher vote count.
