Adjust defaults for whether emails and phones are shared between members of a household

Not often are people sharing emails these days and the default is still that it is shared with members of the household. The same applies for phone numbers. It'd be great to be able to set those defaults based on the type of email/phone that is being entered.

    Or to at least be able to set the box to unchecked by default on the form so end users don't have to remember to uncheck it every time.
