Make the "Confirmation" code table on the "Mark Deceased" form editable, make the field hideable or hard code just 2 values of "Confirmed" and "Unconfirmed" Merged

We are often notified of death by a bank, a lawyer or an obituary, so we are including all options (inc. "Unconfirmed") in the Source code table, as a workaround, and disregarding Confirmation. In not being able to hide this field it causes confusion to users, so even the workaround is less than perfect.
